Ambien is a widespread sleeping drug in the United States, which has recently attracted media attention, especially after such massive night awakenings began to cover more and more people. 

Sleep and food are the two most necessary things for a person, and, as it turned out, they are also interconnected, because the brain regions responsible for these two functions are located very close to each other. Hormones that control our appetite also affect the sleep cycle. When a person does not get enough sleep, these “general hormones” can have an impact on the appetite. In addition, during sleep, the body begins to produce a substance such as leptin, which suppresses hunger during sleep. Many doctors do not believe that the basic substance Zolpidem, which is contained in Ambien, can in any way cross the functions of sleep and hunger, especially in such low doses that are usually prescribed to patients. 

Moreover, it is very difficult to identify patients who eat at night, because some do not suspect such a harmful habit. At night, people seem to be unconscious, so they do not remember anything about it in the morning. Sometimes the only sign of a “night invasion of hunger” is empty boxes and an empty refrigerator. 

Pharmacological action

Ambien is a hypnotic drug of imidazopyridine structure with a rapidly advancing effect. Unlike benzodiazepines, Ambien (active ingredient Zolpidem) is a selective agonist of the omega1-benzodiazepine receptor subclass. 

This may explain the relative absence of muscle relaxant and anticonvulsant effects and the preservation of the deep sleep stage when taking Ambien. Ambien has been shown to shorten sleep time, reduce the latent phase of sleep, prolong overall sleep time, prevent early awakening, and reduce the number and duration of night-time awakenings. Ambien has no sedative effect during the day and is not addictive. 

Indications for use

Situational or long-term sleep disorders: 

– the difficulty of falling asleep; 

– night and early awakenings.

Method of application

Adults under the age of 65 years are prescribed 10 mg (1 pill) per day.

Persons older than 65 years or patients with liver failure are prescribed a dose of 5 mg (1/2 pill) per day. Increasing the dose to 10 mg (1 pill) in this category of patients can only be a last resort.

The maximum daily dose is 10 mg.

The course of treatment should be as short as possible, namely from a few days to 4 weeks (maximum).

With episodic insomnia, the duration of treatment is from 2 to 5 days, with transient – 2-3 weeks.

If you need to extend therapy for more than 4 weeks, you should re-consult the patient.

The drug should be taken immediately before bedtime.

Side effect

From the digestive system: in some cases, abdominal pain, nausea, vomiting, diarrhea are observed.

From the central nervous system: depending on the dose and individual susceptibility (especially in elderly patients) cause dizziness, imbalance, ataxia, headache, daytime drowsiness, distraction, muscle weakness, diplopia, anterograde amnesia, paradoxical or psychopathological reactions.

Allergic reactions: skin rash, itching.

Other: rarely violations of sexual desire are noticed; the development of addiction, the formation of mental and physical dependence (when using the drug for several weeks); ricochet insomnia (when using the drug in high doses or with prolonged use).

Contraindications

– severe respiratory failure;

– night apnea; 

– severe liver failure;

– children and adolescents up to 15 years;

– hypersensitivity to the components of the drug.

The drug is not recommended for use with myasthenia gravis and the simultaneous use of alcohol.

Pregnancy and lactation:

Ambien is contraindicated in the first trimester of pregnancy. 

In the III trimester of pregnancy, the drug can be used in minimal doses, since the use of benzodiazepines and similar drugs in high doses at the end of pregnancy may cause hypotension, hypothermia and respiratory distress syndrome of new-borns, and a few days after birth, the development of withdrawal syndrome in the child.

The concentration of Ambien in breast milk is small. However, if necessary, the use of Ambien during lactation should decide on the termination of breastfeeding.

In experimental studies, there was no teratogenic effect of the drug.

Overdose:

Symptoms: central nervous system depression. In mild cases, deafness and drowsiness are observed; in more severe – ataxia, hypotension, hypotension, respiratory depression, rarely coma. 

Treatment: gastric lavage, artificial vomiting, taking activated charcoal. If necessary, symptomatic therapy in the intensive care unit is required.

Drug interaction:

With the simultaneous use of opioid analgesics, antitussive drugs, barbiturates, antidepressants, sedatives, antihistamines, benzodiazepines, clonidine, neuroleptics, ethanol with Ambien, symptoms of depression of the central nervous system and the respiratory center may develop.

Concomitant use of Ambien with clozapine increases the risk of collapse, stopping blood circulation, and breathing.
