Soma is a skeletal muscle relaxant that affects CNS. It is structurally and pharmacologically similar to meprobamate, mebutamate and tibamate. Its muscle relaxing effect is low and probably results from its sedative action. Soma doesn’t block the neuromuscular transmission. The anticholinergic, antipyretic and analgesic properties of Soma are poorly expressed.

There have not been many reports of Soma overdose, which is partly due to its low efficacy as a muscle relaxant. However, overdose of Soma can be fatal and has almost the same symptoms as meprobamate poisoning. Patients with overdose symptoms should be admitted to the intensive care unit.

Soma (Carisoprodol) is also known as isobamate and isopropyl meprobamate. It is sold under the following brand names: Caprodat (Sweden), Somadril (Denmark, Norway and Sweden), Carisoma (Great Britain), Carisoma Compound (Great Britain — 175 mg of carisoprodol + 350 mg of acetaminophen), Flexartal (France) and Mioxom (Italy).

Dosage

Soma is used as an addition to pain relief and rehabilitation therapy, as well as during physiotherapy to relieve pain associated with skeletal muscle injury. The efficiency of Soma alone has not been studied. Its muscle relaxant effect may result from its sedative action.

The recommended dosage of Soma is 350 mg four times a day (orally). Safe dose of Soma for children have not been established.

The drug is rapidly metabolized by dealkylation, hydroxylation and conjugation. After taking a therapeutic dose, metabolites are formed faster than absorption occurs. The main derivative of Soma is meprobamate. Rats excreted about 7.5% of Soma in the urine and 0.3%- in the feces 48 hours after administration.

Goldberg reports that in the first 20 hours, one of his patients excreted 5 g of the drug in the urine, and another- only 637 mg. However, the method used by this author measured only the total amount of carisoprodol and meprobamate. According to Adams et al., the main metabolite excreted after Soma overdose is meprobamate (along with some hydroxymeprobamate).

Main indications and contraindications

Soma is intended for the treatment of the following diseases and conditions:

  • back spasms that cause chronic pain syndrome (in case of lumbar osteochondrosis);
  • spine surgeries (for example, because of herniated disc);
  • degenerative cervical spine disorders;
  • hypertonia caused by severe neurological disorders (multiple sclerosis, myelopathy and stroke);
  • pathologic vertebral fractures;
  • back injuries;
  • protrusion and herniated disc.

 

Contraindications:

  • drug allergy;
  • chronic kidney disease;
  • children under one year of age;
  • all types of myasthenia gravis (muscle weakness);
  • liver diseases;
  • stomach or peptic ulcer;
  • Parkinson’s disease;

Pregnancy and lactation

Soma is secreted in breast milk, where it reaches a concentration that is 2-4 times lower than in maternal plasma. Based on the results of the animal studies, the drug crosses the placenta.

Side effects of Soma include skin rash, drowsiness, sickness, vomiting, vertigo, flushed skin, extreme tiredness, itching, ataxia, tremor, headache, irritability, sick feeling in the pit of your stomach and drug induced dermatitis. Allergic reaction to meprobamate is also possible. Like other muscle relaxants, Soma can cause drug addiction.
